UPDATED DRUGS AND THEIR DOSAGES FOR CHILDREN IN MG/KG BWT
1. Paracetamol: 10-15mg/kg 4-6hrly
2. Frusemide: 1-2mg/kg dly
3. Ceftriaxone: 80mg/kg dly or in two divided doses For meningitis: 50mg/kg 12hrly or 100mg/kg dly
4. Diazepam: PR – 0.5mg/kg in children stat. IV – 0.25mg/kg in children stat
5. Phenobarbital: IM; 15mg/kg loading dose Oral/IM maintenance: 2.5-5mg/kg. NB: 20mg/kg in infants <2wks IV
6. Phenytoin: 18mg/kg as loading dose IV
7. Quinine: Oral/IM – 10mg/kg 8hrly. IV – 20mg/kg as loading dose, then 10mg/kg bwt 8hrly
8. AS/AQ: 4mg/kg of artesunate and 10mg/kg of amodiaquine dly or in 2 divided doses
9. Erythromycin Oral: 12.5mg/kg 6hrly
10. Azithromycin oral: 20mg/kg dly
11. Ciprofloxacin oral/IV: 10-20mg/ kg per dose 12hrly
12. Metronidazole IV/oral: 7.5mg/kg 8hrly but 10mg/kg in giardiasis
13. Prednisolone oral: 1mg/kg twice dly
14. IV Aminophylline: Loading dose: 6mg/kg slowly over 20mins. Maintenance dose: 5mg/kg up to 6hrly
15. Amoxicillin oral or IV: 25mg/kg twice daily. For pneumonia: 40mg/kg 2x dly
16. IM/IV Ampicillin: 50mg/kg 6hrly
17. IM Artemether: 3.2mg/kg as loading dose then 1.6mg dly
18. IV/IM Artesunate: 2.4mg/kg at 0, 12 and 24hrs followed by 2.4mg/kg dly when necessary
19. Artemether-lumefantrine oral: 2mg/kg artemether plus 12mg/kg lumefantrine twice dly. NB: Don’t give in <5kg or <6months
20. Cefotaxime IV: 50mg/kg 6hrly
21. Cefalexin Oral/IV: 12.5mg/kg 6hrly
22. Chloramphenicol IM/IV/Oral: 20-25mg/kg 6hrly
23. Flucloxacillin IM/IV: 25-50mg 6hrly
24. Cotrimoxazole (septrin) Oral: 4mg/kg trimethoprim + 20mg/kg sulfamethoxazole 2x dly
25. Digoxin Oral: 15mcg/kg loading dose then 5mcg/kg 12hrly of maintenance dose to be started 6hrs after loading dose
26. Fluconazole Oral: 3-6mg/kg
27. Furazolidone oral: 1.25mg/kg 4x dly
28. Gentamicin IM/IV: 2.5mg/kg 8hrly or 7.5mg/kg dly
29. Ibuprofen oral: 5-10mg/kg 6-8hrly
30. Metoclopramide IV/IM/Oral: 0.1-0.2mg/kg 8hrly
31. Morphine: IM; 0.1-0.2mg/kg 4-6hrly. IV; 0.05-0.1mg/kg 4-6hrly
32. Benzathine penicillin IM: 50,000iu/kg dly
33. Benzyl penicillin (X’pen): IM/IV; 50,000iu/kg 6hrly. But 100,000iu/kg 6hrly in meningitis
34. Procaine penicillin IM: 50,000iu/kg dly
35. Potassium chloride IV: 2-4mmol/kg per day
36. Tetracycline oral: 12.5mg/kg 6hrly NB: Stains teeth in children
37. Ketamine: IM loading dose; 5-8mg/kg and 1-2mg/kg for further dose. IV loading dose: 1-2mg/kg and a further 0.5-1mg/kg PRN
